The default implementations are defined in crash_console_helpers.S. The
platforms have to define plat_crash_console_*.

Implemented placeholders for platforms that were missing helpers.

To make software license auditing simpler, use SPDX[0] license
identifiers instead of duplicating the license text in every file.

NOTE: Files that have been imported by FreeBSD have not been modified.

This patch support single core to boot to Linux kernel
through Trusted Firmware.
It also support 32 bit kernel and 64 bit kernel booting.
