      This patch applies the TBBR naming convention to the certificates
      and the corresponding extensions defined by the CoT:
      
          * Certificate UUID names
          * Certificate identifier names
          * OID names
      
      Changes apply to:
      
          * Generic code (variables and defines)
          * The default certificate identifiers provided in the generic
            code
          * Build system
          * ARM platforms port
          * cert_create tool internal definitions
          * fip_create and cert_create tools command line options
          * Documentation
      
      IMPORTANT: this change breaks the compatibility with platforms
      that use TBBR. The platform will need to adapt the identifiers
      and OIDs to the TBBR naming convention introduced by this patch:

      Patch 7e26fe1f deprecates IO specific return definitions in favour
      of standard errno codes. This patch removes those definitions
      and its usage from the IO framework, IO drivers and IO platform
      layer. Following this patch, standard errno codes must be used
      when checking the return value of an IO function.

      Use numbers to identify images instead of names · 16948ae1

      The Trusted firmware code identifies BL images by name. The platform
      port defines a name for each image e.g. the IO framework uses this
      mechanism in the platform function plat_get_image_source(). For
      a given image name, it returns the handle to the image file which
      involves comparing images names. In addition, if the image is
      packaged in a FIP, a name comparison is required to find the UUID
      for the image. This method is not optimal.
      
      This patch changes the interface between the generic and platform
      code with regard to identifying images. The platform port must now
      allocate a unique number (ID) for every image. The generic code will
      use the image ID instead of the name to access its attributes.
      
      As a result, the plat_get_image_source() function now takes an image
      ID as an input parameter. The organisation of data structures within
      the IO framework has been rationalised to use an image ID as an index
      into an array which contains attributes of the image such as UUID and
      name. This prevents the name comparisons.
      
      A new type 'io_uuid_spec_t' has been introduced in the IO framework
      to specify images identified by UUID (i.e. when the image is contained
      in a FIP file). There is no longer need to maintain a look-up table
      [iname_name --> uuid] in the io_fip driver code.
      
      Because image names are no longer mandatory in the platform port, the
      debug messages in the generic code will show the image identifier
      instead of the file name. The platforms that support semihosting to
      load images (i.e. FVP) must provide the file names as definitions
      private to the platform.
      
      The ARM platform ports and documentation have been updated accordingly.
      All ARM platforms reuse the image IDs defined in the platform common
      code. These IDs will be used to access other attributes of an image in
      subsequent patches.
      
      IMPORTANT: applying this patch breaks compatibility for platforms that
      use TF BL1 or BL2 images or the image loading code. The platform port
      must be updated to match the new interface.

      Major update to the FVP platform port to use the common platform code
      in (include/)plat/arm/* and (include/)plat/common/*. This mainly
      consists of removing duplicated code but also introduces some small
      behavioural changes where there was unnecessary variation between the
      FVP and Juno ports. See earlier commit titled `Add common ARM and CSS
      platform code` for details.
      
      Also add support for Foundation FVP version 9.1 during FVP config
      setup to prevent a warning being emitted in the console.

      The intent of io_init() was to allow platform ports to provide
      a data object (io_plat_data_t) to the IO storage framework to
      allocate into. The abstraction was incomplete because io_plat_data_t
      uses a platform defined constant and the IO storage framework
      internally allocates other arrays using platform defined constants.
      
      This change simplifies the implementation by instantiating the
      supporting objects in the IO storage framework itself. There is now
      no need for the platform to call io_init().
      
      The FVP port has been updated accordingly.
      
      THIS CHANGE REQUIRES ALL PLATFORM PORTS THAT USE THE IO STORAGE
      FRAMEWORK TO BE UDPATED.

      Fix the following issues with the console log output:
      
      * Make sure the welcome string is the first thing in the log output
      (during normal boot).
      * Prefix each message with the BL image name so it's clear which
      BL the output is coming from.
      * Ensure all output is wrapped in one of the log output macros so it can
      be easily compiled out if necessary. Change some of the INFO() messages
      to VERBOSE(), especially in the TSP.
      * Create some extra NOTICE() and INFO() messages during cold boot.
      * Remove all usage of \r in log output.

      Previously, platform.h contained many declarations and definitions
      used for different purposes. This file has been split so that:
      
      * Platform definitions used by common code that must be defined
        by the platform are now in platform_def.h. The exact include
        path is exported through $PLAT_INCLUDES in the platform makefile.
      
      * Platform definitions specific to the FVP platform are now in
        /plat/fvp/fvp_def.h.
      
      * Platform API declarations specific to the FVP platform are now
        in /plat/fvp/fvp_private.h.
      
      * The remaining platform API declarations that must be ported by
        each platform are still in platform.h but this file has been
        moved to /include/plat/common since this can be shared by all
        platforms.

      Update code base to remove variables from the .data section,
      mainly by using const static data where possible and adding
      the const specifier as required. Most changes are to the IO
      subsystem, including the framework APIs. The FVP power
      management code is also affected.
      
      Delay initialization of the global static variable,
      next_image_type in bl31_main.c, until it is realy needed.
      Doing this moves the variable from the .data to the .bss
      section.
      
      Also review the IO interface for inconsistencies, using
      uintptr_t where possible instead of void *. Remove the
      io_handle and io_dev_handle typedefs, which were
      unnecessary, replacing instances with uintptr_t.

      Reduce the number of header files included from other header
      files as much as possible without splitting the files. Use forward
      declarations where possible. This allows removal of some unnecessary
      "#ifndef __ASSEMBLY__" statements.
      
      Also, review the .c and .S files for which header files really need
      including and reorder the #include statements alphabetically.

      Add tag names to all unnamed structs in header files. This
      allows forward declaration of structs, which is necessary to
      reduce header file nesting (to be implemented in a subsequent
      commit).
      
      Also change the typedef names across the codebase to use the _t
      suffix to be more conformant with the Linux coding style. The
      coding style actually prefers us not to use typedefs at all but
      this is considered a step too far for Trusted Firmware.
      
      Also change the IO framework structs defintions to use typedef'd
      structs to be consistent with the rest of the codebase.

      Trusted firmware binaries loaded via semihosting used to be
      opened using 'r' mode (i.e. read mode).  This is fine on POSIX
      conforming systems (including Linux) but for Windows it also means
      that the file should be opened in text mode. 'rb' mode must be
      specified instead for binary mode.  On POSIX conforming systems,
      'rb' mode is equivalent to 'r' mode so it does no harm.

      The policy functions for each file to be loaded were implemented by
      copy/pasting one method and then varying the data checked.
      
      This patch creates a generic function to check the policy based on the
      data stored in a table.
      
      This removes the amount of duplicated code but also makes the code
      simpler and more efficient.

      Add Test Secure Payload Dispatcher (TSPD) service · 375f538a

      This patch adds the TSPD service which is responsible for managing
      communication between the non-secure state and the Test Secure Payload
      (TSP) executing in S-EL1.
      
      The TSPD does the following:
      
      1. Determines the location of the TSP (BL3-2) image and passes control
         to it for initialization. This is done by exporting the 'bl32_init()'
         function.
      
      2. Receives a structure containing the various entry points into the TSP
         image as a response to being initialized. The TSPD uses this
         information to determine how the TSP should be entered depending on
         the type of operation.
      
      3. Implements a synchronous mechanism for entering into and returning
         from the TSP image. This mechanism saves the current C runtime
         context on top of the current stack and jumps to the TSP through an
         ERET instruction. The TSP issues an SMC to indicate completion of the
         previous request. The TSPD restores the saved C runtime context and
         resumes TSP execution.
      
      This patch also introduces a Make variable 'SPD' to choose the specific
      SPD to include in the build. By default, no SPDs are included in the
      build.

      The Firmware Image Package (FIP) driver allows for data to be loaded
      from a FIP on platform storage. The FVP supports loading bootloader
      images from a FIP located in NOR FLASH.
      
      The implemented FVP policy states that bootloader images will be
      loaded from a FIP in NOR FLASH if available and fall back to loading
      individual images from semi-hosting.
      
      NOTE:
      - BL3-3(e.g. UEFI) is loaded into DRAM and needs to be configured
        to run from the BL33_BASE address. This is currently set to
        DRAM_BASE+128MB for the FVP.

      The modified implementation uses the IO abstraction rather than
      making direct semi-hosting calls.  The semi-hosting driver is now
      registered for the FVP platform during initialisation of each boot
      stage where it is used.  Additionally, the FVP platform includes a
      straightforward implementation of 'plat_get_image_source' which
      provides a generic means for the 'load_image' function to determine
      how to access the image data.
